If you have retirement or life insurance benefits at Publix, selecting your beneficiaries — the person(s) who will receive your money in the event something happens to you — should be a top priority. That’s why we make it easy for you to make an election and change a beneficiary in just a few steps!
For retirement (PROFIT Plan and 401(k) SMART Plan)
- Log in to PASSport.
- Fill out the PROFIT Plan Beneficiary Designation Form and/or 401(k) SMART Plan Beneficiary Designation Form.
- Print, sign and submit the completed form(s) to the retirement department.
For life insurance (Group Life Insurance Plan and Supplemental Life Insurance Plan)
- For group life insurance, log in to PASSport and fill out the Group Life Insurance Beneficiary Designation Form. Print, sign and submit the completed form to the group benefits department.
- For supplemental life insurance, call Publix Personal Plans toll-free at 1‑888‑374‑6377. Representatives are available Monday through Friday, 9 a.m. to 6 p.m. Eastern time.
