What is a beneficiary?
A beneficiary is the person (or people) you want to receive your benefits when you pass away.
Which benefits should I have a beneficiary for?
The benefits you should make sure you have a beneficiary(ies) for are
- PROFIT Plan, an Employee Stock Ownership Plan (ESOP)
- 401(k) SMART Plan and
- Group Life Insurance Plan.
How do I add or update my beneficiary(ies)?
For retirement — PROFIT Plan and 401(k) SMART Plan:
- Log in to PASSport.
- Fill out the PROFIT Plan Beneficiary Designation Form and/or 401(k) SMART Plan Beneficiary Designation Form.
- Follow the instructions on the form to complete it and send to the appropriate department.
- If you want to view or update your beneficiary(ies) in the future, visit the PROFIT Plan or 401(k) SMART Plan page on PASSport under the Financial tab, then select Retirement & Stock.
For your Group Life Insurance Plan:
- Log in to PASSport.
- Fill out the Group Life Insurance Beneficiary Designation Form.
- Follow the instructions on the form to complete it and send to the appropriate department.
- If you want to view or update your beneficiary(ies) in the future, visit the Group Life Insurance page on PASSport under the Financial tab, then select Life Insurance.
